If Medical Issues Exist (as indicated by "yes" response(s) on the
PAR-Q & You form):

- PARmed-X for Pregnancy. Please click here for a copy of this document.
- Letter to physicians completing the PARmed-X or PARmed-X for pregnancy forms are available by clicking here. Please also print and attach the PARmed-X or PARmed-X for pregnancy form. This is a special note to Health Professionals completing the forms by clicking here.
